Description:
Position Purpose:
The Care Transitions Case Manager collaborates with physicians, social services, nurses, community agencies and other members of the care team to define care options and resources, to plan cost-effective quality care and to achieve optimal quality outcomes. The Care Transitions Case Manager is primarily responsible for selection of and ensuring the timely and seamless clinical transition of patients through medically appropriate levels of care.
What you will do:
Coordinates/facilitates patient care progression throughout the continuum while advocating for the patients best possible outcome
Works collaboratively and maintains active communication through interdisciplinary rounds with physicians, nursing and other members of the multidisciplinary team to effect timely, appropriate patient management
Coordinates with social workers to ensure a warm handoff with caregivers at next site of care including but not limited to health coaches, home care agencies, and nursing homes or skilled facilities
Proactively identifies, triages, and assists in resolving delays and obstacles to discharge. Responsible for assuring through case management assessment, as well as early and ongoing discharge planning by collaborating with patients, families, physicians, payors, and providers across the continuum of care
Conduct face-to- face assessments and document primary and back-up discharge plan on all patients
Serves as a resource person for families, patients, employees and other health care professionals, needing information on Mercy Medical Center-North Iowa services and community services(communication)
Assists in creating processes to anticipate and address specific situations within the department, including post-acute resource referral lists, denials, readmissions, and Length of Stay
Applies quality improvement methods and techniques to improve case management processes to maximize costs and quality benefits for MercyOne North Iowa Medical Center
Hours/Schedule:
Baylor Position. Working 0600-1830 Saturday and Sunday; Holiday rotation (weekend holidays).
Minimum Qualifications:
Current and valid license to practice as Registered Nurses in Iowa, BSN or approved BA is required or willing to obtain within three years of employment
Five years Clinical RN experience required
Case Management Certification preferred
Diverse knowledge of community services, resources and programs.
Previous case management experience preferred.
Special Skills and Competencies:
Excellent interpersonal communication and negotiation skills.
Strong analytical, data management and PC skills.
Ability to establish and maintain working relationships.
Ability to work independently and exercise sound judgment in interactions with physicians, payors, associates and patients/families.
High degree of initiative and judgment.
Ability to work flexible hours.
Able to set and organize own work priorities and adapt to changes and interruptions.
Must provide proof of Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status.
Completes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.
