Job description

How you will change lives…

Responsible for residential shift leadership and performs, coaches and has oversight of Rehab. Tech I and IIs performance of prescribed/therapist directed clinical intervention techniques to patients in the rehabilitation setting to facilitate patient practice and increase safe and independent functioning in individual goal areas, including mobility, activities of daily living, communication, cognition, health management, psychosocial, leisure and community integration. Serve as a mentor to Rehab. Tech. I and II staff through the development of effective therapeutic working relationships with patients and their support network.

Monitor and provide assistance with personal care, protection and supervision, as needed to residents that is consistent with the mission, vision and values of Hope Network. 

 

The Rehabilitation Technician III assists in the following areas of team guidance and client personal care & development:

Leadership

  • Supports in training of newly hired staff.
  • Assists with documentation for various business purposes.
  • Specializes in specific healthcare need(s) as assigned by program manager

 

Safety

  • Monitors and ensures safety 24/7; this may be eyes-on, one-on-one, 15 or 30 minute checks, etc.
  • Supports in crisis prevention, intervention, and resolution.

 

Relationship Building

  • Establishes collaborative relationships built on trust and empathy through consistent and meaningful communication.
  • Fosters a spirit of partnership with clients, the community, and other professionals.

 

Activities and Daily Living

  • Helps build effective skills for problem-solving, communication, and independence.
  • Assists clients in identifying and/or participating in social and leisure interests such as transporting/escorting to stores, concerts, movies, and in-house social events (games, barbeques, etc.)
  • Supports clients with Activities of Daily Living, including but not limited to: getting to and from places, cooking, dressing and undressing, bathing and showering, combing hair, trimming nails, restroom assistance.
  • Aids clients with bowel and bladder programs, feeding (G-tubes), wound care, and transfers.

 

Health

  • Monitors vitals for any medical conditions or illnesses.
  • Promotes positive behavior.
  • Attends medical appointments and ensures accurate information is relayed between medical provider and care team.
  • Understands, reads, and assists in the implementation of medical, behavioral, and/or clinical team care plans and provides accurate, timely, and descriptive documentation of such.
  • Oversees client medication needs.

 

Community Integration

  • Connects clients with opportunities in their community to develop relationships with others.
  • Supports employment, educational, and volunteer interests and goals.

We are Hopeful you have…

  • Certification as a CNA or a bachelor’s degree from an accredited university or institution of higher learning (preferred) or High School Diploma or G.E.D with three years of experience, or one year as an HNNR Rehab. Tech. and have completed all HNNR Core and Advanced Medical Rehab Tech Skills requirements.
  • One year experience as a Hope Network Rehabilitation Technician or two years of experience in a human service agency.
  • Work experience in a licensed residential setting preferred.
  • Valid driver’s license.
  • Ability to lift 50 pounds.
  • Ability to become certified in CPR/First Aid.
  • Commitment to scheduled shifts, assigned duties, and performance expectations.
  • Basic computer skills.
