Position Summary

The Senior Social Worker – PACE is a senior level position that supports the PACE program by evaluating the MSW department on an ongoing basis to continually improve participant care, services, population health, and health plan management.

Essential Functions

  • Review new regulations and requirements for the PACE program and identify what new regulations impact the PACE Social Work department. 
  • Evaluate current processes, procedures and policies and implement change to adhere to new requirements as needed.
  • Orient new PACE MSWs to Social Work department policies, procedures, and guidelines. 
  • Create agenda and facilitate routine department meetings for training and regulatory oversight purposes
  • Perform routine clinical audits for assessment and Plan of Care congruency and identify gaps.  Educate individual MSWs for any identified opportunities for improvement.
  • Evaluate the PACE program MSW caseload quarterly and make changes to assignments based on acuity, location, and individual participant factors.
  • Evaluate educational needs for the Social Work department and coordinate training. 
  • Develop relationships with service area adult family homes and assisted livings to facilitate communication and placement opportunities. 
  • Oversee disenrollment policy and procedure management by individual MSWs and provide support and guidance as needed.  Report outcomes to PACE Compliance department.
  • Meet regularly with Clinical Director to report on department functioning re: compliance to regulations and participant service expectations
  • Provide clinical supervision to MSW staff seeking clinical licensure or other MSWs in the Care Continuum per senior leader request.

Qualifications

  • Master’s degree in Social Work required
  • Clinically licensed for Social Work in the state of WA
  • Certificate for supervision within one year of hire
  • Minimum five (5) years experience working with an Interdisciplinary Team, preferably seven (7).
  • Experience with geriatrics or disabled adults for at least 5 years required

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $75,941.00 - $109,262.00 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
