Job Overview

We are seeking a dedicated Bilingual Nurse Care Planner (Korean) to join our esteemed Long-Term Care Management team. In this remote position, you will play a crucial role in delivering vital care management services to economically disadvantaged and culturally diverse older adults, as well as disabled adults and their caregivers. Your expertise will significantly contribute to enabling clients to remain safely in their homes, thereby preventing unnecessary institutionalization.

Key Responsibilities

  • Conduct thorough assessments of new clients to establish program eligibility, including verification of nursing home eligibility and identification of medical, physical, and psychological requirements through initial and ongoing evaluations.
  • Formulate and execute individualized care plans that receive approval from the program supervisor, ensuring clear communication of these plans to clients and their representatives.
  • Provide continuous health education to non-clinical staff while offering essential resources to clients and their families related to self-care, hygiene, and nutrition.
  • Engage actively in weekly team case conferences, presenting complex healthcare cases to facilitate discussion and learning.
  • Communicate with clients’ physicians as necessary to address health concerns raised during assessments.
  • Collaborate closely with various stakeholders, including staff, clients, family members, home health agencies, and community services to ensure clients receive necessary services and maintain a safe living environment at lower costs than skilled nursing facilities.
  • Maintain comprehensive monthly documentation of client interactions and pertinent clinical records promptly.
  • Ensure accurate documentation of services rendered, adhering to timely authorizations and verifications according to contract guidelines.
  • Participate in quality assurance initiatives, including chart reviews and training sessions for new hires, while contributing to audits as required.
  • Stay informed about Medicare and Medi-Cal policies, as well as federal and state legislation impacting health and social services, while actively pursuing professional development opportunities.

Qualifications

  • Nursing degree required; Bachelor's degree in Nursing (BSN) preferred.
  • Must possess an active California RN license in good standing.
  • A minimum of 3 years of experience in health assessments, care planning, and case documentation, ideally with older adults. Previous experience with home visits and knowledge of mental health and substance abuse issues is advantageous.
  • Willingness to travel within the program's service area for home visits, accompanied by a valid driver's license, automobile insurance, and reliable transportation.
  • Strong organizational, verbal, and written communication skills, with the ability to function independently as well as collaboratively within a team. A keen attention to detail and a customer-focused mindset are essential.
  • Bilingual proficiency in English and Korean (proficiency will be assessed).
  • Availability from Monday to Friday, 8 AM to 5 PM, with flexibility for extended hours when necessary.
