Working Title: Head of Community Health

FLSA Status: Exempt

Posting Salary Range: $66,306 -$76,915

Office Location: Remote within North Carolina.

What We’re Looking For

Trillium Health Resources has a career opening for Head of Community Health to join our team! The Head of Community Health will be responsible for developing, implementing, and evaluating strategies and interventions to advance health across 46 counties in North Carolina. This position will represent Trillium Health Resources and work collaboratively within the Community Health and Innovation Department, community-based teams and organizations, healthcare providers, public health agencies, and other stakeholders to identify and address the root causes of health disparities to improve health for all people and communities across predominantly rural eastern and central North Carolina.

On a typical day, you might:

  • Establish and maintain strong relationships with community-based organizations, faith-based groups, policymakers, key decision-makers and other key stakeholders in rural communities
  • Conduct community needs assessments to identify health disparities and prioritize interventions; Facilitate community forums and/or focus groups to gather input on strengths, solutions, barriers and feedback on initiatives to improve health 
  • Develop and implement strategies to engage populations that have been underserved and systemically discriminated against in community health planning and decision-making, work to remove systemic barriers to health and healthcare access
  • Design and implement culturally relevant health education, outreach programs, and evidence-based programs and interventions to address health disparities, including those related to social determinants of health (e.g. poverty alleviation, safe and affordable housing, education and literacy, access to fresh food)
  • Collaborate with healthcare providers to improve access to and quality of care for populations that have been underserved and systemically discriminated against
  • Manage program budgets, ensure efficient use of resources, evaluate program effectiveness and make recommendations for continuous improvement

Qualifications: 

Education/Experience: 
 High School diploma/GED and five (5) years of experience in a field focused on services related to community health including counseling, education, community-based or nonprofit program management and operations to include experience with tribes, public speaking, grant writing, community collaboration, outreach, career or health fairs.

OR

Associate’s degree and three (3) years of experience in a field focused on services related to community health including counseling, education, community-based or nonprofit program management and operations to include experience with tribes, public speaking, grant writing, community collaboration, outreach, career or health fairs.

OR

Bachelor’s degree and one (1) year of experience in a field focused on services related to community health including counseling, education, community-based or nonprofit program management and operations to include experience with tribes, public speaking, grant writing, community collaboration, outreach, career or health fairs.

OR 

Equivalent combination of education/experience.

Preferred Education: 
 Advanced degree in a field related to community health and access such as Counseling, Education, American Indian Studies, Minority studies, Sociology, Social Justice, Public Health, Public Administration, Social work, or another relevant human services area is preferred

 

Preferred Experience: 
 Supervision of two (2) or more employees 
 Experience working with federal and/or state Tribal communities on matters related to partnering to improve health and opportunities and stakeholder engagement

License/Certification: 
 Must have a valid driver’s license

Location: 
 Remote within North Carolina. 
 Must reside in NC to be considered for remote status. 
 Must be able to travel to a Trillium office location and within catchment as required.
