Gateway Rehab Center (GRC) has an outstanding opportunity for a Care Coordinator! THIS IS A MOBILE POSITION! Starting pay is $19/Hour! 


The ideal candidate MUST live in Allegheny County or surrounding counties/communities to the Pittsburgh, PA region.  

 

Our Care Coordinator provides mobile care management and peer support, if applicable, to identified individuals who meet the criteria for enrollment into the GRC - Center of Excellence/COE program. As Coordinator, you will be providing assessment of non-treatment needs and barriers, developing support plans to address and monitor needs, and working on an interdisciplinary team to ensure successful recovery plans and support for individuals.  


Other responsibilities include but not limited to the following: assessment of needs, maintaining a caseload by providing onsite and mobile care management to eligible individuals, and referral source communication throughout program involvement until discharge. This position receives regular supervision, instruction and training.


Responsibilities

  • Performs assessment of non-treatment needs on identified patients.
  • Maintains a caseload of eligible individuals to provide care management wherever the individual presents and/or is in need.
  • Assist patients in meeting all identified non-treatment needs, such as assisting with medical, legal and other necessary appointments, as well as providing referrals for housing and other resources needed.
  • Completes all documentation in patient’s charts regarding care coordination and peer engagement related activities.
  • Initiates comprehensive verbal follow up with the identified patients before, during, at discharge and following discharge. 
  • Maintains continual assessment of the unique needs of the patient towards development of individual support plans.
  • Networks and provide feedback with referral sources and outside treatment providers.
  • Communicates with clinical staff and other Gateway departments for case management purposes and provides follow-up with the referral source.
  • Serves as a member of the program’s interdisciplinary team.
  • Participates in group supervision, individual supervision, case consultations, and all team meetings. 
  • Attends GRC mandatory in-services and trainings.

Knowledge, Skills, and Abilities 

  • Excellent people and organizational skills.
  • Strong written and verbal communication skills. Teamwork within the care coordination department and with other departments is critical to success in this position.
  • Knowledge and experience in chemical dependency treatment preferred.
  • Computer literacy is required.


Requirements
  • Bachelor’s Degree in a health-related field.
  • Certified Recovery Specialist certification through the Pennsylvania Certification Board is preferred.

Additional Requirements

  • Valid driver’s license and favorable driving record.
  • Pass PA Criminal Background Check
  • Obtain PA Child Abuse and FBI Fingerprinting Clearances
  • Pass Drug Screen
  • TB Test  

Work Conditions

  • Working conditions include work in agency and community settings with occasional evening and/or weekend hours and some local travel.
  • Minimal physical demands.
  • Significant mental demands including interviewing and investigating; ability to work independently; and frequent communication and maintenance of relationships with referral sources.
