Description

The Social Worker will work in conjunction with the Nurse Case Managers, coordinating care of the member population served by ProgenyHealth based on the CMSA national standards for case management practice.  The Social Worker will demonstrate strengths in working independently as well as collaboratively within a team environment. An understanding of ProgenyHealth goals and outcome measures is important to the accountability of the Social Worker and the success of the member-focused Specialty Care Management Programs.


Responsibilities Include:

  • Completes initial and ongoing assessments within established timeframes to identify needs and barriers.
  • •Uses engagement models and tools to maintain high engagement rates.
  • Assesses, plans, and implements care strategies that are member centric and directed towards meeting the needs of the member. 
  • Builds and maintains relationships with members and healthcare providers to obtain input when developing an individualized plan of care that addresses functional and psycho-social barriers to optimal outcomes.
  • Identifies and coordinates referrals for social services programs; including financial, psychosocial, community, state, and federal supportive services; monitors activities to ensure that services are actually being delivered and meets the needs of the member.
  • Conducts calls to members to provide them with an overview of our services and works to develop open communication. 
  • Assesses members’ behavioral and psychosocial status as appropriate and connects them to available health plan or community resources to assist with overcoming challenges. 
  • Communicates with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members.
  • Identifies any barriers to care and advocates for member as needed to ensure the needs and choices are fully represented and supported by the healthcare team.
  • Provides education to members regarding benefits and services and how to access needed care.
  • Participates in Interdisciplinary and client rounds and assists with coordination of safe transitions of care.


Requirements

Qualifications: 

  • Minimum of 2 years background working in managed care or relevant clinical environment, with maternity population.
  • Must have a valid unrestricted Social Worker license (LCSW) to practice in Florida and the ability to obtain licensure in other state(s) when applicable.
  • Master’s degree in Social Work (MSW) required.
  • CCM certification preferred upon hire; required within the timeframe specified by company policy.
  • Experience working with a Medicaid population is preferred.
  • A background in effecting change to overcome social determinants of health, including behavioral health and substance abuse disorder.
  • Experience with data entry into a database or EHR required. Must be computer literate.
  • Excellent communication skills and organizational ability are required. Must be self-motivated and willing to learn multiple tasks. Must be well organized and able to prioritize tasks.
  • Must demonstrate accuracy in spelling and documentation.
  • Must have excellent interpersonal skills, telephone etiquette, and maintain positive communication at all times.
