As a licensed clinician, the Clinical Program Manager is accountable for leading clinical programs, clinical performance, and/or contract performance initiatives, ensuring alignment at a regional and/or system level.
Essential Functions Of The Role
Partners with internal and external stakeholders to meet contractual and/or regulatory obligations.
Proactively identifies, plans, implements, evaluates, and monitors quality improvement and performance improvement initiatives.
Significantly contributes to or runs system and/or regional initiatives by gathering data, conducting research, maintaining records, tracking issues and barriers, evaluating the impact of interventions, coordinating activities, and executing plans to resolve issues.
Researches and maintains knowledge of current evidence-based practices and works with multidisciplinary teams to build a replicable model for evidence-based clinical programs and guidelines. Develops program tools and resources such as guidelines, training/education materials, and enhancement requirements.
Acts as a credible change agent and Subject Matter Expert (SME) related to program management, process improvement, and clinical and contract performance.
Acts as a liaison across the care continuum to multidisciplinary teams and internal/external stakeholders.
