A caregiver in this position remotely between the hours of 7:00AM - 5:00PM, with local travel to the CCAC and Independence FHC locations as needed. We are seeking candidates who reside in the Greater NE OH only!!

A caregiver who excels in this role will:  

  • Develop, implement, manage, and maintain operational systems and activities. 
  • Serve as a liaison with all levels of the organization and outside community. 
  • Manage multiple priorities and projects with competing deadlines. 
  • Allocate time to meet completion requirements. 
  • Serve as a coach and mentor for other positions in the department. 
  • Review and monitor adherence to Human Resources policies and corporate compliance procedures. 
  • Interpret (KPIs) for payor contracts and population health.
  • Work with healthcare data to identify trends and monitor outcomes.

Minimum qualifications for the ideal future caregiver include:  

  • Bachelor's degree in business or related field.O
  • Offset:  Associate’s degree and five years of experience.
  • Offset:  Master’s degree and two years of experience.
  • Minimum of three years project experience in the assigned area, which may include progressively responsible experience supporting a clinical system and/or data/financial analysis, etc.

Preferred qualifications for the ideal future caregiver include:  

  • Master’s Degree
  • Familiarity with health system navigation and the patient journey/experience
  • Knowledge of statistical and financial analysis, primary research and business plan development
  • Knowledge of health IT systems like population health platforms
  • Familiarity with risk stratification, care coordination and value-based care models

Physical Requirements:

  • Ability to communicate and exchange accurate information. 
  • Working in a stationary position for extended periods.  
  • Working with physical records
  • Operating a computer or other office equipment.  
  • In some locations, ability to travel throughout the hospital system.  
  • In some locations ability to move up to 25 lbs.
