Responsibilities
Performs patient assessment and provides care.
Educates patient on their condition, prescribed medications, nursing
Interventions and care plans.
Participates in the coordination of services with other disciplines and community resources as needed.
Identifies and acts on changes in the patient's condition.
Evaluates and revises care plans/assessments based on changes in patients and/or their environments.
Documents all assessments and interventions.
Collects specimens as required (including phlebotomy).
Administers medications including IV medications as ordered.
Utilizes and instructs patient/family on the use of the telehealth tablet used for biometric data.
Participates in regular case conferences and in-service or special case orientations as needed and participates in on-call rotation.
Performs other duties as assigned.
Qualifications
Associate science in nursing required, BSN preferred.
Currently licensed to practice in the state in which you are required to work.
Two years of RN experience, three to five years of inpatient RN experience in med/surg preferred.
Experience providing RN services in the home, community or clinic is preferred.
BLS.
