The Nurse Care and Case Management (CCM) Manager will oversee and lead the day-to-day efforts of our CCM Nurse Care Managers. The Manager will play a key role in helping nurses achieve individual and team-level goals, with a focus on delivering high quality and efficient care and case management to our members.
As we grow, the Nurse Care and Case Management (CCM) Manager will support the professional growth and development of their direct reports, while also fostering team engagement and enhancing overall performance.
Responsibilities:Actively track and manage performance for all clinicians under your leadership, ensuring timely and high-quality care deliveryManage team members to meet performance metrics, including volumes, customer commitments, service line agreements, and performance guaranteesCollaborate with the Quality and Clinical Excellence (CE) teams to promote the highest level of care and service excellencePartner with Human Resources Business Partners (TAO) to develop formal remediation plans when necessarySupport the growth and development of individual clinicians, helping them achieve their professional goalsServe as a bridge between clinical leadership and clinicians to implement workflow changes and drive continuous improvementCommunicate team vision, updates on organizational processes, and champion changes that impact the navigation teamRepresent the team in cross-functional collaborations and partner with service line leaders to enhance workflowsEnsure that care and case management services are in alignment with the organization's values and objectivesCommunicate openly and collaboratively with nurses, leadership, and cross-functional teamsDevelop and manage policies and procedures that align with organizational goalsDemonstrate innovative and creative thinking, identifying opportunities for improvement in workflows and processesMake timely decisions and effectively manage change in a dynamic environmentLead and engage others in driving success, fostering a culture of accountability and high performanceNavigate difficult conversations with professionalism and care when necessaryQualifications:BSN requiredCCM Certification requiredActive Nursing License, in good standing with state's BON.5+ years of nursing experience required2+ years care management leadership requiredStrong organizational and team-building skillsAbility to work efficiently, be detail-oriented, and adapt in a rapidly changing environmentExceptional communication skills with a positive and respectful approachProven experience leading teams or initiatives with measurable resultsProficiency in technology and data-driven decision-makingProven ability to manage change and adapt in a fast-paced, evolving organizationShift: Monday-Friday 9AM-6PM local time (Preference for for those in MST/PST timezone)Physical/Cognitive RequirementsPrompt and regular attendance at assigned work location.Capability to remain seated in a stationary position for prolonged periods.Eye-hand coordination and manual dexterity to operate keyboard, computer and other office-related equipment.No heavy lifting is expected, though occasional exertion of about 20 lbs of force (e.g., lifting a computer \ laptop) may be required.Capability to work with leadership, employees, and members in an appropriate manner.This range reflects the minimum and maximum target for new hire salaries for candidates based on their respective Zone. Zone A - $89,580 - $116,640 + equity + benefits
Zone B - $98,538 - $128,106 + equity + benefits
Zone C - $107,496 - $139,752 + equity + benefits
Zone D - $116,454 - $151,398 + equity + benefits
Starting base salary for you will depend on several job-related factors, unique to each candidate, which may include education; training; skills; years and depth of experience; certifications and licensure; our needs; internal peer equity; organizational considerations; and understanding of geographic and market data. Compensation structures and ranges are tailored to each zone's unique market conditions to ensure that all employees receive fair and great compensation package based on their roles and locations.
