Key Responsibilities
Deliver direct patient care to diverse age groups and health conditions.
Perform comprehensive assessments and maintain accurate patient histories.
Develop and execute individualized care plans tailored to patient needs.
Administer prescribed medications and treatments.
Monitor vital signs and document any changes in patient conditions.
Work collaboratively with interdisciplinary teams to ensure complete patient care.
Educate patients and families on health management and disease prevention.
Ensure all patient records and documentation are precise and current.
Qualifications And Requirements
Graduate of an accredited school of professional nursing. BSN, PHN preferred.
Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.
Current CPR certification.
Strong communication and interpersonal skills.
Active RN license in the state of Arizona or a multistate license.
Per Diem: Candidates applying for per diem must have 6 months of home health experience as an RN.
Full-Time: No RN and home health experience is required.
