Job description

 

Salary Range: $67.1400 - $103.2200 /hour. Actual compensation may vary based on geographic location, work experience, skill level, and education.

 

About Hoag On-Demand
Hoag is launching a virtual on-demand care platform for treating urgent-like and acute care symptoms for patients 13 years of age or older in June 2025. It will further the Hoag mission of bringing accessible, convenient, high-quality care to the community.
Patients will login or create an account, share their concerns, complete a brief health profile, sign consent forms, select their pharmacy, and get in queue immediately to see a provider. They will also be able to select a time to get in queue within a 24-hour period, at a time that is
convenient for them. After the visit, a Care Coordinator will help coordinate follow-up care with other providers or services within Hoag. The clinical team will consist of NPs, Care Coordinators, and an Administrator, providing care Monday through Sunday from 7am to 11pm, with an expansion to 24-hour care later in the year.

 

Primary Duties and Responsibilities

 

The Telehealth Nurse Practitioner provides high-quality advanced nursing care through the virtual on-demand care platform, working in accordance with professional regulatory standards and within the scope of their licensure. This role is designed to deliver telehealth services for
urgent concerns ensuring seamless access to patient-centered care. Responsibilities include conducting remote patient evaluations and assessments via telehealth, documenting and charting patient encounters promptly and accurately within the electronic health record (EHR), and providing follow-up care recommendations such as patient education and preventive measures.


The Nurse Practitioner will also prescribe medications and coordinate care with other Hoag providers to ensure comprehensive treatment. They will recognize and address ethical, legal, and medical dilemmas in the practice and consult with appropriate resources when necessary. Care will be delivered independently or under direct/indirect supervision of a supervising physician to a diverse population across various age groups 13 years of age or older. This fully remote, per diem position offers flexible hours, including evenings, weekends, and holidays, based on patient demand, with the potential for on-call responsibilities. Privileges may include ordering laboratory and diagnostic tests, interpreting results, and collaborating with multidisciplinary teams to ensure optimal patient outcomes. Also may furnish medications (non-narcotic) and durable medical equipment (DME), controlled substances III, IV, V with appropriate licensure/certification, and controlled substances II with appropriate licensure/certification. May order blood products, laboratory tests, radiological, ultrasound, nuclear medicine, and neurological studies, oxygen therapy, respiratory treatment, and pulmonary tests.

 

Education and Experience
Required:
• Master’s in Nursing (MSN) or DNP required. Completion of proctoring by Medical Staff Office.
Advanced clinical knowledge and skills in the care and management of patients.
• Demonstrates effective communication, ability to prioritize clinical and operational needs,
interpersonal skills, organizational ability, knowledge of proper body mechanics, safety measures and
infection control and ability to follow instructions. Demonstrates clinical proficiency and efficiency.
• Must have a passion to be a part of a team dedicated to changing healthcare access and
delivery.
Preferred:
• Three (3) years minimum clinical experience with rapid clinical decision-making and triage in a
relevant specialty (i.e. urgent care, family, emergency) for the pediatric and adult population.
• Comfortable working as a single provider and seeing patients of all ages.
• Previous telemedicine or virtual care experience.
• Previous experience with prescribing non-narcotic medications
• Knowledge of electronic medical record systems, specifically EPIC, patient registration,
scheduling, and insurance verification processes.

Licenses Required
• Current licensure as a Registered Nurse and Nurse Practitioner in good standing in the State of
California.
