Responsibilities
Notifies supervisor or designee as needed when documentation is delinquent from physicians, clinicians or other sources.
Maintains filing of medical records, physicians' orders, lab reports, progress notes and clinical notes for active and discharged patients in EMR.
Monitors and processes physician orders through HCHB or Forcura.
Monitors outgoing failed faxes and troubleshoots to rectify.
Performs timely communications and provision of required patient records and details to admitting facilities and physicians for hospice patients.
Handles outreach and coordinates with designated care center staff when hand delivery of orders is required due to lack of timely follow up from physician office or facility.
Performs and supports IDG (Inter Disciplinary Group) console preparation for assigned branches.
Conducts clerical audits on admission, recertification, and discharge charts and processes according to company policy.

Qualifications
High school diploma or GED.
Two years of office or related experience, preferably in a healthcare environment.
Demonstrated knowledge of the appropriate skills for communicating with individuals of all ages, especially the geriatric population.
Excellent interpersonal, verbal and written communication skills.
Strong computer and software skills.
Working knowledge of personnel management, record keeping and office administration.
Reliable internet service and ability to office from home with minimal disruption to the workday.
Preferred
Associate's degree in business or related field.
