As a Bilingual Specialty Health Coach-Diabetes at DarioHealth you will support, encourage and educate members with chronic conditions as they work towards reaching their health goals. Using your coaching and clinical background you will play a key role in supporting members and improving health outcomes. In addition, you will serve as SME for diabetes on coaching team.

Responsibilities

Coach a caseload of Dario members across all Dario conditions using multi-modal commuication.
In addition, serve as coaching team SME for Diabetes: coach Dario members who are needing additional support related to Diabetes condition, support training and content needs on occasional basis.
Provide training and assistance to members on how to use their Dario device(s) and navigate in the DarioHealth app.
Address Social Determinants of Health (SDoH) needs and provide resources to members who are facing barriers and challenges to optimal health and wellness.
Support and assist members to create, build upon, and sustain new habits.
Be member centric – meeting the member where they are in the process.
You understand and value the coaching scope of practice as it applies to chronic disease management.
Other duties as assigned.

Requirements

CDE, RN or Clinical License.
Bachelor’s degree or higher
5+ years of experience in health coaching, health education, nutrition or behavioral health. Previous experience with chronic disease management education, specifically diabetes, a plus. Familiarity with motivational interviewing and behavior change theories.
Open-minded, non-judgmental and compassionate - you know how hard it can be to change old habits, but you never give up on people.
Health Coach Certification preferred
Bilingual in English/Spanish preferred
Demonstrated effective organizational and communication skills
Independent, creative self-starter, flexible, comfortable with ambiguity, and thrive in a fast-paced work environment that encourages critical thinking and creative problem-solving.
Be results-oriented with demonstrated time management skills
Ability to learn, analyze, understand and convey complex information
Demonstrated technical aptitude. Proficiency with technology and web-based applications
