Job description

LTSis seeking an experienced Transition Manager responsible for planning, coordinating, and managing the transition of health readiness programs, services, and systems. This role ensures that health operations, personnel, and resources are seamlessly moved from one phase of a program, system, or operational structure to another while ensuring compliance with DoD policies and health readiness goals.

Duties & Responsibilities

  • Develop and implement transition strategies for health readiness programs, ensuring alignment with DoD objectives and health standards.
  • Manage the transition process for health systems, ensuring smooth integration of new technologies, policies, and operational procedures.
  • Work with stakeholders across different levels of the DoD, military services, contractors, and external organizations to coordinate transitions effectively.
  • Oversee the transition and integration of new health IT systems or upgrades to existing systems, ensuring compatibility with DoD health databases and records.
  • Identify potential risks during transition processes and create mitigation strategies.
  • Monitor transition progress, resolving issues quickly to minimize disruption to health services and readiness.
  • Ensure that all transition activities comply with relevant DoD regulations, directives, and health readiness policies.
  • Prepare reports and presentations for senior leadership, summarizing transition progress, outcomes, and areas of improvement.
  • Lead and coordinate teams responsible for various aspects of the transition, ensuring that each member's responsibilities are clear and well-managed.

Required Skills & Qualifications

  • Must be a U.S. citizen.
  • A bachelor’s degree in a related field (e.g., healthcare administration, business management, or public health) is required.
  • Experience in health readiness, health systems management, or project management, with at least 2 years in a leadership or coordination role.
  • Experience with the DoD healthcare systems, military health programs, or federal health regulations is strongly preferred.
  • Strong project management skills, including the ability to plan, execute, and monitor complex projects.
  • Excellent communication, leadership, and interpersonal skills.
  • In-depth knowledge of DoD health policies and regulations.
  • Ability to manage multiple priorities and meet deadlines in a high-pressure environment.
  • Ability to work effectively with military and civilian personnel in a collaborative environment.
  • Proficient in health IT systems and tools.
