Job Summary
This position is responsible for conducting care management and health education programs for customers on government health care programs. Accountabilities include gathering, analyzing and providing data for regulatory reports. Perform assessments, care planning, and other care reviews in the member/family home and telephonically. This position will represent the company to members, providers, and state regulators
JOB REQUIREMENTS:
- Registered Nurse (RN), with 2 years direct clinical care to the consumer in a clinical setting or unrestricted Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), Licensed Independent Social Worker (LISW), or Licensed Mental Health Counselor (LMHC), with 2 years of clinical practice experience
- Current, valid, unrestricted license in the state of New Mexico (or reciprocity). For compact licensee changing permanent residence to state of operations, you must obtain active, unrestricted RN licensure in the state of operations within 90 days of hire.
- Experience handling medical management programs and health educations programs in an independent manner.
- Knowledge of the health and wellness marketplace and employer trends.
- Verbal and written communication skills including discussing medical needs with members and interfacing with internal staff/management and external vendors and community resources.
- Analytical experience including medical data analysis.
- PC proficiency to include Word, Excel, and PowerPoint, database experience and Web based applications.
PREFERRED JOB REQUIREMENTS:
- Plus 1 year’s wellness or managed care experience presenting clinical issues with members/physicians.
- 3 years clinical experience.
- Justice facility experience.
- Patient education experience.
- Condition Management experience.
- Bilingual in English and Spanish.
- Experience in managing complex or catastrophic cases.
- Certification in Case Management, Training, Project Management or nationally recognized health care certification.
This is a Telecommuter position traveling to meet with members in these New Mexico areas (Santa Fe, Las Vegas, Albuquerque and Espanola) locations.
**Bilingual (English and Spanish) and Care Coordination experience preferred**

** Masters-level Behavioral Health Professionals: Please note that you MUST be licensed to practice at the independent level for this role. The # of years of clinical experience required for this role is post licensure (# of years) at the independent level**
Work Schedule: Monday through Friday 8 AM to 5 PM MST (with the flexibility to work later hours and weekends – to accommodate Members’ needs).

The Compensation/Salary offered for this Position will vary depending on Job-related skills, education, knowledge and experience. This role aligns with an Annual Performance Incentive (Bonus) plan.

The compensation offered will vary depending on your job-related skills, education, knowledge, and experience. This role aligns with an annual incentive bonus plan subject to the terms and the conditions of the plan.
