What We’re Looking For
Trillium Health Resources has a career opening for an RCT Director of Clinical Care Management to join our Care Management Team. This position provides clinical leadership and supervision to our Regional Care Managers and their Care Teams at Trillium Health Resources. The Director is responsible for ensuring that our Care Teams are assessing and coordinating appropriate services for our members while monitoring that they are receiving quality care. The primary responsibilities include providing clinical oversight to the Tailored Care Management team, ensuring the team is supporting members to achieve better health outcomes, and the team is maintaining positive collaborative relationships with the medical team, treatment providers and other people who support our members.
On a typical day, you might:
Review Performance Dashboard to monitor daily productivity of the team
Coordinate initiatives to promote compliance with standards and regulations.
Monitor case mix and caseload size of care teams and adjust resources as needed to ensure effective delivery of service.
Collaborate with Regional Managers to ensure care plans are properly written and identify the appropriate services.
Provide clinical oversight to Care Management activities for members and their families.
Provide direct supervision to RCT Regional Managers.
Review and analyze reports and dashboard and use the information for designing improvement strategies.
Participate on the clinical panel for facilitation of weekly clinical staffings, multidisciplinary care rounds and case conferences.

Qualifications:
Required Education:
A Master’s degree in Psychology, Social Work, Counseling, Human Services, Psychiatric or Nursing.
Required Experience:
Minimum of three (3) years in leadership experience in care management or care coordination within the public health sector or Managed Care.
Preferred Experience:
Direct Supervision of two (2) or more employees.
Experience as a Manager or Director to four (4) or more employees.
Required License/Certification:
Fully licensed by the State of North Carolina as a PhD, Licensed Clinical Social, Worker (LCSW), Licensed Clinical Mental Health Counselor (LCMHC), Licensed, Marriage and Family Therapist (LMFT), Licensed, Clinical Addiction Specialist (LCAS), or Licensed Psychological Associate (LPA).
Must have a valid driver’s license
Location:
Remote within North Carolina.
Must reside in North Carolina to be considered for remote status.
