Job description

Job Overview

We are seeking a dedicated Community Support Specialist to join our compassionate team at Compass Health Network. This full-time position is essential for delivering high-quality care to the communities we serve. The ideal candidate will work Monday through Friday from 10:00 am to 7:00 pm, with a rotating Saturday schedule every five weeks from 10:00 am to 2:00 pm. To attract top talent, we are pleased to offer a sign-on bonus of \(500, with an additional \)500 bonus available for Qualified Mental Health Professionals.

Key Responsibilities

  • Deliver individualized and group educational services based on a motivational, strengths-based approach in collaboration with clients and their families.
  • Assist clients in identifying their strengths and preferences, contributing to the development of personalized Wellness Plans.
  • Provide a holistic perspective in service delivery, focusing on the Eight Dimensions of Wellness.
  • Facilitate access to public services and maintain contact with hospitalized clients to aid in discharge planning.
  • Provide education and skill-building support in areas such as daily living skills, budgeting, and symptom management.
  • Act as a liaison within the integrated health team, relaying vital information about client wellness to all team members involved in care.
  • Complete all documentation within the required timeframe and follow up with clients to ensure engagement and support in their recovery journey.

Required Skills

  • Strong communication and interpersonal skills for effective client and team interactions.
  • Ability to provide tailored educational support and advocate for client needs.
  • Competence in documentation and maintaining client records accurately.
  • Understanding of the Eight Dimensions of Wellness and how to implement them in practice.
  • Valid driver’s license and the ability to assist clients with transportation needs as clinically indicated.

Qualifications

  • High School diploma or GED is required; an Associate's or Bachelor's degree in a human services-related field is preferred.
  • Proven experience working with individuals facing psychiatric, substance use, and/or developmental disabilities, with varying experience requirements based on educational qualifications.
  • A valid driver’s license and compliance with agency-established minimum automobile coverage are required.
