Mountain View Hospital is looking for a Charge Entry Specialist/OR Scrub Tech to join our team!

JOB SUMMARY:       

The Charge Entry Specialist will be responsible for Patient charges/assisting with charge information updates. The Operating Room Scrub assists the surgeon at the sterile field   by providing appropriate and adequate sterile instruments and other supplies during the operative procedures.  Maintains the surgical field through aseptic practices.  Works under the supervision of the Surgery Team Leader as a member of the perioperative team. Assists in the nursing care and preparation of the surgical patient for surgery and performs routine and delegated duties according to the standards of practice and policies of the facility and department.  Facilitates and demonstrates cooperative behavior with colleagues and supervisors at all times.

DUTIES AND RESPONSIBILITIES:

The RN Pre-Op/Admitting:

  1. Maintains area in a state of readiness at all times, ensuring that all necessary equipment is available and in good working order, with age specific supplies ready for use as needed.
  2. Assesses physical and emotional needs of patients and implements appropriate nursing interventions.  Completes the nursing pre-assessment record, including confirmation of the following:
    1. Allergies.
    1. Medications taken at home.
    2. Dentures, contact lenses and glasses.
    3. Pertinent medical conditions listed.
    4. Pertinent past medical history listed.
    5. Confirmations of consent and operative site.
    6. Confirmation of responsible party.
    7. Confirmation of ordered labs/test results on chart.
  3. Respects the patient’s right to privacy, dignity and confidentiality.
  4. Assists the patient in undressing if needed and stores belongings appropriately.
  5. Assesses or performs the following:
    1. Vital signs.
    2. Weight and height.
    3. Accu-Check for blood sugar, if applicable.
    4. Pregnancy test, if applicable.
    5. Starts IV on all patients, except those who have an order not to start an IV. Utilizes anesthesia protocol regarding IV fluids and catheter size.
    6. Pain level.
  6. Performs pre-operative instruction and explanation of procedures and patient’s rights.
  7. Ensures that patients are seen by the anesthesia provider prior to being taken to the O.R.
  8. Sets priorities to facilitate the timely and efficient flow of patents throughout the day.
  9. Carries out physician/anesthesia orders.
  10. Records pertinent patient information on the nurse’s notes.
  11. Demonstrates an economic use of equipment and supplies.
  12. Maintains confidentiality of all pertinent patient care information to assure patient’s rights are protected.
  13. Demonstrates commitment to excellence in patient, family and physician relations.
  14. Communicates appropriate information to other members of the healthcare team in a timely manner.
  15. Acts as the nursing advocate for the patient by upholding patient rights.
  16. Participates in overall Pre-Op/PACU Department functions, including, but not limited to:
    1. Follows appropriate Pre-Op/Admitting policies/procedures.
    2. Training new staff as needed.
    3. Attends and participates in staff meetings.
    4. Attends annual education in-services as required.
    5. Is knowledgeable about and responsive to emergency situations, i.e. cardiac arrest, malignant hyperthermia, drug reaction, fire, disaster, etc.
  17. Complies with attendance requirements.
  18. Demonstrates an awareness of safe conditions and safe acts as related to hospital employment.
  19. Maintains professional competency through appropriate continuing education.
  20. Adheres to and practices MVH Compliance Program and participates in Performance Improvement activities.
  21. Performs other related duties incidental to the work described herein.
  22. If assigned as Pre-Op Team Leader, performs the following opening and closing duties:

Opening:

    1. Counts narcotics.
    1. Closing:
      1. Checks with PACU i.e. staffing needs.
      2. Stocks the blanket warmer.
      3. Readies rooms: gurneys made up, O2 tanks >500psi.
      4. Restocks IV trays. Restocks room carts.

The PACU, Phase I RN:

  1. The Charge Entry Specialist:       

  2. Will be responsible for entering surgical charges into patient accounts as marked by the surgical staff on the preference cards.
  3. Will enter these charges the day after the surgical date.
  4. For those charges that can’t be located, will contact the appropriate departments to create Charge Master additions.
  5. Will work with Materials Department to audit charge amounts to verify accuracy. 
  6. Will be responsible for coding and charge entry for anesthesia services as well as assuring accuracy in surgery procedure charges. 
  7. The OR Surgery Technologist:

  8. Is responsible for the setting up and performance of surgical cases by assembly of equipment and supplies according to physician preference cards.
  9. Is responsible for the setting up and performance of surgical cases by opening sterile pack, supplies, and instruments according to sterile technique.
  10. Prepares sterile field, including sponge, needle, and instrument counts when needed.  Ensures the sterility of supplies and instrumentations used during operative procedures.
  11. Works cooperatively in the cleanup of the operating room following surgery, including the proper disposal of used supplies and sharps, the cleaning and decontamination of instruments, equipment and operating room.
  12. Handles specimens according to proper procedure.
  13. May act as an assistant to the circulating nurse in preparing the patient for surgery, positioning the patient in the appropriate position, and moving the patient to and from operating table.  Assists the circulator in other duties as directed.
  14. Demonstrates understanding and utilization of principles of asepsis in all duties.
  15. Uses supplies conservatively and efficiently.
  16. During surgical procedures:
    1. Passes instruments to the surgeon and the assistant.
    2. Is attentive to the operative area by being alert for breaks in technique.
    3. Maintains an attitude of willingness to assist with the surgical procedure.
    4. Is aware of the location of all instruments, needles, sponges, and specimens.
  17. Assists the circulator with responsibilities for intra-operative safety related to infection control, prevention of exposure to blood-borne pathogens, and handling hazardous material.
  18. Prepares soiled instruments for cleaning.  Assists as needed in the decontamination and reprocessing of instruments.
  19. Works cooperatively with other staff in putting unused supplies and equipment away and restocking rooms.
  20. Adheres to and practices MVH Compliance Program and participates in Performance Improvement activities.
  21. Performs on-call duties for emergency surgeries during after-duty-hours and on weekends on a rotating basis.
  22. Maintains professional competency through appropriate continuing education.
  23. Is fully knowledgeable of policies and procedures specific to the department of surgical services.
  24. Performs job duties in agreement with AST Standards of Practice.
  25. Assists with any other duties as deemed necessary.

Qualifications

Education/Certification:  Graduate of an accredited Surgical Technologist Program. Certification as a Surgical Technologist (CST) preferred. Current BLS, required if working with patients. If working remotely, BLS not required

 Experience:  Computer experience required. Operating room experience required.

 Equipment/Technology:  Ability to work with Cerner system

Language/Communication:  Must have the ability to effectively read, write, and speak English.

Mathematical Skills:  Basic math skills.                      

Mental Capabilities:  Ability to work in a fast-paced environment. Can adapt to rapidly changing workloads due to changes in the surgery census. Can frequently and consistently make sound judgments.

Interpersonal:  Works well with others. 

Performance:  Can work well under pressure and meet multiple and competing deadlines.
