UW Medical Center-Montlake – Social Work

has an outstanding opportunity for a

SOCIAL WORK ASSISTANT 2.

WORK SCHEDULE


  • 20 hours per week
  • Day Shifts - alternating Wednesdays and every Thursday/Friday
  • This position is fully-remote



POSITION HIGHLIGHTS


  • Work closely with Physicians, Nursing, Social Work, other members of the medical team and the patient to arrange follow-up appointments post-discharge, transmit the treatment plan to primary care physicians and other following services, and assist patients with locating community resources and understanding their benefits
  • This position serves as a member of the University of Washington Social Work and Care Coordination department



PRIMARY JOB RESPONSIBILITIES


  • Assist in arranging for Skilled Nursing and other post-acute care placements post-discharge, based on information obtained from clinical staff
  • Assist in arranging home services, such as home health, home infusion, wound vacs, or in-home caregivers
  • Assist in arranging practical needs and emergency assistance, such as transportation, parking, food or local lodging
  • Assist the social worker and the patient/family in locating non-medical housing (respite, shelter)
  • Research and update community resource information as needed
  • Assist patients and families in accessing educational resources relating to the medical and psychosocial impacts of their illness



REQUIRED POSITION QUALIFICATIONS

Bachelor of Social Work or a Bachelor's Degree in the behavioral sciences AND two years experience as a Social Work Assistant I or equivalent. OR Associate of Arts Degree in the behavioral sciences AND four years of work experience as a Social Work Assistant I or equivalent.

Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.
