Job description

Position Summary

Directly reporting to the Project Manager, the Community Health Administration Coordinator I, will establish a close relationship with patients and clinic site coordinators to assist with referrals, follow-ups, and completing applications for services. The Community Health Administration Coordinator I will be fluent in reading, writing, and speaking in the Spanish language, and will work in collaboration with the Project Manager in determining eligibility for healthcare services to the uninsured and will coordinate transportation services for patients as needed. Additionally, Community Health Administration Coordinator I will work closely with the Project Manager to serve in a secretarial role to maintain the clerical functions of the Project for patient scheduling, obtaining, and recording patient biometric data, and electronic health record management and documentation.

The Community Health Administration Coordinator I will assist in health literacy efforts to ensure that the health information provided is culturally and linguistically appropriate for the population served. This position will be responsible for designing and maintaining the clinic website, to include biographies of the clinicians, description of services offered, patient office visit schedules for both San Diego and Los Angeles, and flyers and descriptions of upcoming health fairs and events.  The Community Health Administration Coordinator I will collaborate with faculty in the assignment of students and setting up the health fair booth and collection of patient intake information at health fairs.  This position will schedule and maintain the minutes for all committee meetings; will develop and maintain OneDrive file folders containing student and faculty information; and will serve as the facilitator in the use and implementation of immersive technologies for online instruction. 

Essential Functions:

  • Responsible for establishing trusting relationships with patients and their families and serve as an interpreter for Spanish-speaking patients.
  • Follows – up with patients via phone calls, and clinic site visits.
  • Assists patients to set personal health related goals and attend appointments.
  • Provides referrals for services to community agencies as appropriate.
  • Helps patients connect with transportation resources.
  • Exhibits excellent working relations with patients, visitors, and staff.
  • Works closely with healthcare teams to help ensure that patients have comprehensive and coordinated care plans.
  • Knowledgeable about community resources appropriate for the needs of patients/families.
  • Responsible for providing consistent communication to the Project Manager to evaluate.
  • patient/family status, ensuring that provided information, and reports clearly describe progress.
  • Acts as a patient advocate and liaison between the patient/family and community service agencies.
  • Performs basic secretarial functions to include patient scheduling, documentation of patient intake information in the EHR, translate patient handouts into the Spanish language.
  • Attends regular staff meetings, training, off-site healthcare events, and other meetings, as requested.
  • Manages assigned caseload of patients.
  • Maintains HIPPA compliance at all times.
  • Maintains minutes of Project committee meetings.
  • Actively participates in faculty/student/staff meetings.
  • Collects and records clinical data needed for federal quarterly reports.
  • Sets up the clinic site at all locations to include exam table, tablet, laptop, printer, clinic supplies, etc.
  • Attends and staffs the booth for all health fairs and community events to include hoisting the canopy, tables, chairs, patient documents, etc.
  • Serves as a Facilitator during simulation sessions to engage students in the use of immersive technologies.
  • Assists with planning and presenting at community events in both Los Angeles and San Diego.
  • Have a working knowledge of and experience in the use of extended reality technologies.

Requirements:

Education & Experience:

  • Baccalaureate Degree in Public Health or related field required.
  • Medical Technologist, Paramedic, or Certified Nursing Assistant preferred.
  • Medical terminology and/or background preferred.
  • CPR certification or BCLS certification required

Location: Remote

Travel: Travel up to 75% to ensure clinical practice consistency across all clinic locations.

Candidate receiving offers will be offered a salary/pay rate commensurate with experience that vary based on a candidate’s qualifications, skills, and competencies.  Absent exceptional circumstances, candidates will be offered a salary within this range for this position. The minimum salary will be offered based on the minimum exemption threshold based on state of residency.
