Job description

Frontera Health is developing a tech-enabled platform to provide pediatric therapies for rural families in need. Our platform leverages AI/ML to establish the foundational video-based data model for early intervention and developmental disorders. By working with parents and caregivers in rural communities, we aim to increase access to care, improve health equity, and deliver personalized treatment for better outcomes.

Frontera Health New Mexico seeks a dedicated individual passionate about our community, with a natural inclination for helping others. As a Community Health Worker (CHW), you'll collaborate closely with pediatric clients, caregivers, and our care team as you work to increase access to ABA services and drive better care outcomes. 

Our aim is to break down barriers hindering community members from accessing ABA services and resources. CHWs play a pivotal role in identifying and overcoming community, cultural, and educational obstacles within the healthcare system, ensuring all individuals have access to high quality and timely ABA therapy services.

About the CHW Role:

In this role, you will operate within the New Mexico Department of Health Community Health Worker Scope of Work, which includes but may not be limited to the following services:

  • Community outreach
  • Community & cultural liaison
  • System navigation & care coordination & case management 
  • Home based support
  • Health promotion & training
  • Community assessment & mobilization
  • Clinical support
  • Translation

 

Essential Duties:

  • Engaging Clients on Frontera’s Waitlist including but not limited to:
    • Referral coordination  
    • Patient education
    • Client & family onboarding
    • Coordinating assessments 
  • Engaging Clients in Ongoing Services 
    • Client & family orientation 
    • Client scheduling 
    • Engaging & supporting clients and families who have put services “on pause” or who are on our “hold list”
    • Engaging & supporting clients & families with frequent cancellations; removing barriers to accessing services with the goal of supporting the continuation of ongoing services

 

In addition to the above, you will:

  • Operate within local, state, and federal regulations, as well as company policies and procedures.
  • Operate within the New Mexico Department of Health Community Health Worker Scope of Work

Requirements:

  • A Community Health Worker certification from the NMDOH Office of Community Health Workers.  There are two paths to obtaining your CHW certification:
    • Applicants without CHW Experience
      • Complete Core Competency Training
      • Apply for CHW Certification through NMDOH
      • Candidates must have CHW Certification before beginning a contract with Frontera Health
    • Applicants with CHW Experience 
      • Grandfathered in & can apply for CHW Certification through NMDOH (specific criteria required to apply)
        • Criteria: 
          • Receive their state certification without completing Core Competency Training if the following criteria is met
            • 18 years of age 
            • Verification of proficiency in the core competencies through training or experience 
            • Two letters of reference 
            • 2000 hours of work or volunteer experience as a CHW in the two (2) years prior to application or at least half-time paid or volunteer employment as a CHW in the last five (5) years prior to application
  • Excellent communication and interpersonal skills with a dedication to family-centered care.
  • Position is eligible for 75% remote work from home, however will require 25% travel to our Frontera Health New Mexico regions 
  • MSW or LCSW candidates are strongly encouraged to apply although these are not required qualifications 
  • We are seeking a candidate who is bilingual in English and Spanish, capable of effectively communicating in both languages.
