Job description

Location: Remote work from home-must live in SC

SHIFT: M-F 8-5, No OT

Duration: 4+ month contract

*We Need 2 RN's that have good computer skills and committed to the job and attendance policy.

Walk me through the day to day responsibilities of this the role and a description of the project:
  • Transition of Care Case Management for Medicaid members, telephonic outreaches to facilities and members. Assist facility staff with discharge planning efforts for members.
Describe the performance expectations/metrics for this individual and their team:
  • Outreaches to members after discharge from hospitalizations within 10 business days of discharge.

Job Description: Position Purpose:
Develops, assesses, and facilitates complex care management activities for primarily physical needs members to provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families.

Education/Experience:
Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2 – 4 years of related experience.


License/Certification:
RN - Registered Nurse - State Licensure and/or Compact State Licensure required.
Required Skills/Experience: Preferred Skills/ Experience:
1. 2+years of nursing experience 1. Case Management
2. Case management experienced preferred, not required. 2.
3. Great computer skills and knowledge of navigating the systems 3.
4. Committment
Education Requirement: RN license (SC) Education Preferred: BSN preferred but not required
Software Skills Required: MS office – Outlook, Word, Excel, Teams
