Position Summary

The Home Health Face to Face Coordinator is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Home Health Face to Face Coordinator will be responsible for inquiring and gathering information from the EMR, physicians, referral sources, agency staff, and other stakeholders to ensure face-to-face demands are met in accordance with payer requirements.


Position Specific Responsibilities

  • Reviews the EMR for attached documents and reviews any F2F encounter notes as well as the SOC, diagnoses, and the physician on the Plan of Care.
  • Procures, organizes, and maintains the documents necessary for F2F clearance.
  • Processes electronic workflow, coordination notes, and administrative tasks within the required timeframes.
  • Scans all received F2F encounter notes to the document management system to initiate the indexing process.
  • Documents all actions in the EMR using an appropriate Coordination Note type.
  • Engages the appropriate team member for assistance when all avenues to acquire a F2F encounter note have been exhausted.
  • Reports concerns and suspected incidences of non-compliance to the Home Health Clinical Supervisor – Face to Face.
  • Participates in monitoring and auditing activities and investigations, and implementing quality improvement processes, as required
  • Maintains the confidentiality of patient and organization information at all times.
  • Performs other duties as assigned.


Education and/or Experience

  • High school diploma or GED required.
  • Prior use of Electronic Medical Records (EMR) required.
  • Minimum of one (1) year of experience in a home care, hospice, personal care services, or medical service setting preferred
  • Experience in dealing with a variety of customers in healthcare, including physicians, clinicians, and healthcare facilities preferred.


Skills

  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, colleagues, investors, and external parties. Excellent written and verbal communications.
  • Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy.


Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 25 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
