FLSA Status: Exempt

Posting Salary Range: $57,436 - $72,653

Office Location:  Remote in or within 75 miles of Trillium’s Mid-State Region (Anson, Guilford, Montgomery, Randolph and Richmond Counties)

What We’re Looking For

The Complex Transitional Care Occupational Therapy Assistant (OTA) is responsible for providing input for Complex Care Coordination targeting those with chronic, unresolved, or complex physical, behavioral health and social determinant needs. This includes providing care planning with foundations in national evidenced based and informed standards to provide whole person care. The OTA completes required documentation/paperwork/tasks in the Trillium software platform according to established timelines.

On a typical day, you might:

  • Screen Trillium members who may have identified needs with behavioral health, physical health, co-occurring, co-morbid or multi-morbid conditions. 
  • Monitors the Care Plan. 
  • Completes Complex Transitional Care Functional Screening and other Occupational Therapy Screening Tools as needed. 
  • Evaluates home environment. Identifies equipment needs. 
  • Identifies equipment needs.
  • Provides education.
  • Participates in multidisciplinary care team.

Qualifications: 

Education: Associates degree - Occupational Therapy

Experience: Associate degree in Occupational Therapy and 4 (four) years documented experience working with the co-occurring of physical health and behavioral health, physical health, or chronic condition population, OR   Equivalent combination of education/experience.

 Preferred Experience: Preferably in all 3 areas above.

License/Certification: Fully licensed by the North Carolina Board of Occupational Therapy as an Occupational Therapy Assistant (OTA) required.

Must have a valid driver’s license

Must reside in or within 75 miles of Trillium’s Mid-State Region (Anson, Guilford, Montgomery, Randolph and Richmond Counties)
