As an IKC Nurse Practitioner (NP) you will support some of our most complex patients, assisting them in navigating a challenging healthcare system. You’ll focus on delivering holistic, integrated care to improve the quality of life for our kidney patients. You’ll work autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, consistent, cost-effective care. 

Position Details

  • Location: Hybrid-Remote. Work from home and Travel within an assigned geography including Dialysis Clinics and Nephrology Practices.

  • Complete Comprehensive Health Assessments

  • Manage complex patients, focused on reducing hospitalizations and improved health outcomes

  • Monday- Friday schedule with the ability to accommodate patient and nephrology partner’s availability. Must be flexible.

  • Full-Time self-managed schedule

  • No nights, weekends and on-call!

Requirements

  • Passion for delivering care to patients with complex, chronic illnesses

  • Minimum of two (2) years' experience as a Nurse Practitioner (NP)

  • Current unrestricted NP license to practice in the state

  • Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire

  • Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification

  • Intermediate computer skills and proficiency in MS Word, Excel & Outlook required

  • Home Office, High Speed Internet and Reliable Transportation Required.

Preferred Qualifications

  • Preferred experience working in Value Based Care or Population Health including documenting and coding from a MA (Medicare Advantage) risk perspective and conducting Health Risk Assessments on high risk/chronically ill patients.

  • 5+ years’ experience as a Family Nurse Practitioner (FNP), Primary Care Nurse Practitioner (NP), or experience in Internal Medicine, Cardiology, Nephrology or Case Management

  • Spanish-speaking preferred in some markets

  • Experience in managing chronic diseases (CHF, diabetes, hypertension, diabetes)

  • Manage pop. Health (hospitalization)

Salary/ Wage Range

$110,100.00 - $161,700.00 / year

Compensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies and experience and may fall outside of the range shown.
