Responsibilities:

The Home Infusion Clinical Pharmacist participates in the daily operation of home infusion services by providing all aspects of pharmaceutical care to alternate site patients including procuring, compounding, dispensing of all medications and supplies within the pharmacy scope of services and within state, federal and regulatory laws and standards.  Clinical activities will include patient education, therapy management, clinical monitoring, nurse education and direct communications with patient/caregiver, nurse and provider.   Position requires participation in the Quality Assurance and Quality Improvement Plan.

Qualifications:

EDUCATION, EXPERIENCE, TRAINING

 

Required qualifications:

 

1.  Active pharmacist license in the state of Illinois
2.  Bachelor’s degree in Pharmacy or Doctor of Pharmacy (PharmD)
3.  At least 1 year of experience in hospital or home infusion pharmacy
4.  At least 1 year of experience in sterile compounding
5.  Critical thinking, decisive judgment, and ability to work independently are required.
6.  Excellent communication, organizational, and customer service skills are required.
 

Preferred qualification:

 

1.  Knowledge in 340B program operations and compliance
2.  Proficiency in Caretend electronic medical records (EMR) system
3.  Knowledge of outpatient pharmacy revenue cycle as it relates to prescription and medical benefits
