Nightingale Clinical Care is looking for a Full-Time Nurse Practitioner (NP) Manager to join our Integrated Care team.  The Nurse Practitioner Manager leads and manages Nightingale nurse practitioners (NPs) in the Sacramento, CA market. This role will be 25% in field and 10% over night travel monthly.

In this role you will provide oversight, coaching, supervision and onboarding of NPs to ensure that quality, cost-effective care is provided for chronic kidney disease (CKD) and/or end-stage kidney disease (ESKD) patients. You will participate in population health management and overall financial viability of the program. This role functions autonomously, yet in partnership with operational and clinical leadership on quality initiatives, outcomes and performance measurements. Managers will have a small patient roster, and flexes within reason, with the needs of the business such as providing coverage for open positions as needed.

ESSENTIAL FUNCTION

  • Serve as the clinical lead for a team of NPs, and oversee their work; including providing in the field supervision and coaching. Ensure NP quality and productivity, including documenting findings, appropriate diagnoses, closing Plan of Care gaps, and accurate coding.  Support NP hiring efforts and oversee onboarding of new NPs. Provide ongoing clinical guidance and training to NPs to improve understanding of CKD, ESKD, and commonly associated chronic conditions.  

  • Serve as key leader of clinical and operational strategy working with Medical Officer and Operations leadership. Identify opportunities to streamline NP workflow and focus management intensity on high risk, high-utilization members. 

  • Lead complex case reviews and timely support of complex case escalations of high utilization and high-risk members working with a team of specialized resources and patient’s care team, and payor resources. 

  • Know and comply with Nightingale Clinical Care, PC policies and procedures in accordance with State and Federal regulations for patient care and services (CMS, HIPAA, OSHA, or other governing agencies); Standards necessary to maintain organizational accreditation (such as NCQA); and, State or Federal regulations of Injury Prevention and Safety Training Program

  • Other duties as assigned.

  • A valid driver’s license – NCC Nurse Practitioners typically need to drive their own automobiles to get from one location to another.  You’ll need a valid driver’s license within the state you are applying for work, as well as valid automobile insurance and an acceptable driving record.

PREFERRED EXPERIENCE

  • 4-6 years of NP Experience; experience in population health, Primary/family care, and/or nephrology a plus.

  • Master’s degree in Nursing.

  • At least 1-year experience in supervision or leadership role

  • Current license to practice in the state of practice

  • NP National Credentialing

  • Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire

  • Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification

  • Excellent verbal and written communication skills

  • Demonstrated proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation regarding the needs and care of chronic disease

  • Deals with confidential information and/or issues using discretion and judgment

  • Intermediate computer skills and proficiency in MS Word, Excel, and Outlook

  • Excellent organizational skills and attention to detail.

Salary Range:

$160,000-$170,000
