Job description

Homebase Medical is getting back to basics. Our mission is to optimize the quality of life for homebound older adults, allowing them to live independently and with dignity in their own homes. We make house calls – enabling access to quality wrap-around services for homebound older adults with complex health needs while improving the quality of their lives, right at home.

We offer an array of medical and chronic care services and we’re backed by SCAN, a not-for-profit organization that has a history of supporting and improving the health and independence of older adults for over 45 years.

Homebase Medical is growing in the San Diego area - Chula Vista/Imperial Beach specifically. We are currently searching for 1 experienced Bilingual Spanish Nurse Practitioners or Physician Assistants to engage in Medicare Wellness Visits for part-time hours for an 11-month contract at our contracted partner clinics..

**Target start date is Dec 2nd for training/onboarding and go live January 2**

**This is a full-time position working in our partner clinics at the following schedule for approximately 11 months:

Chula Vista clinic: Monday - Thursday 8AM to 4PM Lunch 12-1.
Imperial Beach clinic: Friday 8:30 to 5:30 Lunch 12-1.

Responsibilities include:

Perform Wellness Visits in partner primary care clinics
Drive to designated facility locations in your assigned geographic area.
Review patient medical and family history.
Review of current providers and prescriptions.
Perform height, weight, blood pressure, and other routine measurements.
Deliver personalized health advice.
Review risk factors and potential treatment options.
Exhibit stellar bedside manner with the ability to communicate complex medical information with simplicity and kindness.
Provide health education to patients. Work with patients in the prevention of illness, promotion of health, and management of patient's adjustment to chronic health problems.
Provide referrals to appropriate medical resources.
Utilize our EMR system to document patient visit/interaction accurately and timely.
Knowledge of HCC coding.
Other duties as assigned.

Qualifications and experience:

Graduate of an accredited Physician Assistant program or Nurse Practitioner program required.
Current, unrestricted NP or PA license to practice in California, including DEA license.
Minimum of 1 years of experience in primary care, hospice, or urgent care/outpatient setting.
Minimum of 1 year of previous experience utilizing an EMR system.
Good working knowledge of HCC coding.
Strong computer skills including MS Office and Outlook is strongly desired.
Great verbal and written communication skills
Current drivers’ license, auto insurance and reliable transportation
