The Position:
Position General Summary:
The Education Transition Support Specialist, Florida Scholars Academy is responsible for coordinating the transition process from Florida Scholars Academy to the home county school district. This position collaborates with the facility case management staff and FSA support services team to ensure a smooth transition from FSA to the student’s home school district.
Essential Position Functions:
- Disseminate calendar invites to appropriate FSA personnel.
- Attend virtual transition or community reentry team meetings in place of an absent or vacant paraprofessional for their area.
- Collaborate with school counselor and paraprofessional to ensure all transition meetings are attended by FSA staff, documentation deadlines are met, and documents are completed within the JJIS system according to DJJ guidelines and procedures.
- Continual communication about student educational progress and educational goals with facility transition and case management staff.
- Facilitate student transition from Florida Scholars Academy to FLEX home education.
- Utilize a variety of programs and software applications, as appropriate, to maintain student database.
- Any other academic related topic established at the discretion of the Superintendent of Schools, Florida Scholars Academy or Regional Director, Florida Scholars Academy.
- Meet professional obligations through efficient work habits such as, meeting deadlines, honoring schedules, coordinating resources and meetings in an effective and timely manner, and demonstrating respect for others.

Minimum Requirements:
Education/Licensure/Certification:
- Associate’s degree in a related field or an equivalent combination of education and experience.
Experience:
- Three years’ experience working with students identified as at-risk and school placement.
- Two to three years’ experience working with school records, course transcripts, student transition plans, student assessment scores, and student grades.
Knowledge, Skills, and Abilities (KSA's):
- Knowledge of placement options: Home District or FLEX Home School
- Knowledge of working with school records, including but not limited to, graduation requirements, transcripts, Individualized Education Program (IEP), transition plans, assessment scores, and grades in progress.
