Job Overview

We are looking for a dedicated and empathetic Bilingual Nurse Care Planner to join our team in a remote capacity. In this pivotal role, you will manage long-term care for economically disadvantaged, culturally diverse older and disabled adults along with their caregivers. Your contributions will be essential in ensuring our clients can safely remain in their homes, significantly reducing the risk of premature institutionalization.

Key Responsibilities

  • Conduct comprehensive assessments for new clients to establish eligibility for our program and approve nursing home eligibility, while regularly reassessing to determine ongoing medical, physical, and psychological needs.
  • Collaborate with clients and their representatives to create and execute individualized care plans that address their unique requirements and preferences.
  • Provide continuous health education to non-clinical staff and deliver guidance to clients and their families regarding self-care, hygiene, and nutrition.
  • Engage in weekly team case conferences, presenting complex cases and discussing intricate healthcare issues to enhance collective knowledge within the team.
  • Liaise with clients' physicians as necessary to resolve health concerns identified during assessments.
  • Coordinate resources effectively among clients, families, physicians, community services, and other professionals to guarantee appropriate service delivery and maintain a safe living environment.
  • Ensure timely and accurate documentation of client interactions, services rendered, and purchase of service records.
  • Participate in regular quality assurance reviews and audits, contributing to training initiatives that support continuous improvement and onboarding of new employees.
  • Undertake additional responsibilities as assigned to ensure effective team support and client care.

Required Skills

  • Strong organizational, verbal, and written communication skills.
  • Ability to work both independently and collaboratively within a team.
  • Attention to detail and a client-centered approach.
  • Bilingual proficiency in English/Korean (a proficiency test will be administered).
  • Possession of a valid driver's license, automobile insurance, and reliable transportation, with the ability to travel over 50% of the time.

Qualifications

  • A nursing degree is required; a Bachelor's degree in Nursing (BSN) is preferred.
  • A current and active California RN license in good standing.
  • A minimum of three years of experience in health assessments, care planning, and case documentation, with a preference for experience working with older adults and conducting home visits.
