Humana is seeking a Manager, Care Coaching who has primary responsibility for supporting the work of Care Management staff in the assigned location or region. The Manager, Care Coaching is responsible for providing daily leadership and mentoring to staff. The associate in this position works to ensure that services are delivered in a manner that demonstrates Humana values and the Family Care program values of access, choice, quality, and cost effectiveness.
The Manager provides coaching to staff to ensure policies and procedures are appropriately implemented so that service coordination for members supports individual outcomes in the most effective and cost-effective manner. The Manager also demonstrates Family Care’s mission, vision and values in everyday interactions and work.
- Mentor and support the work of Field Nurses and Care Coaches (or IDT) to ensure the Family Care benefit is delivered in compliance with the DHS contract, providing a positive member experience, all while demonstrating Humana’s leadership standards.
- Serve as a resource to clarify process and policy clarification to IDT when addressing programmatic, member, and associate specific challenges.
- Identify and coordinate learning needs; arrange for mentorship, provide clear expectations, facilitate orientation and ongoing learning to build capacity, self-reliance and knowledge.
- Assess and coach staff on the IDT core competencies and performance standards.
- Oversee the quality of care management service delivery.
- Utilize reports, assess process and review documentation.
- Assess utilization and the quality of care management for each member served.
- Participate in cross departmental collaboration to ensure the quality and effectiveness of provider services.
- Engage, educate and develop community partners and internal and external stakeholders.
- Support the work and development of IDT associates.
- Create effective teams by assisting with recruiting, selecting, orienting, and training associates.

Required Qualifications
- Bachelor’s degree in human services or a related field or associate degree in nursing.
- 1 year of management experience in the human services field.
- 3 years of experience working in the Family Care program.
Preferred Qualifications
- Master’s degree in public administration, social work or related field.
- 5 years of experience working with individuals with intellectual/developmental disabilities, physical disabilities or frail elders.
Additional Information
- Workstyle: Home. Home workstyle is defined as remote but will use Humana office space on an as needed basis for collaboration and other face-to-face needs.
- Work Location: must reside in WI, preferably in La Crosse County or the surrounding area.
- Travel: at least once/month for in person visits in La Crosse County area with potential for more travel.
- Typical Work Days/Hours: Monday – Friday, 8:00 am – 4:30 pm CST
- Direct Reports: approximately 12-14
WAH Requirements
To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria:
- At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is recommended; wireless, wired cable or DSL connection is suggested.
- Satellite, cellular and microwave connection can be used only if approved by leadership.
- Employees who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for their internet expense.
- Humana will provide Home or Hybrid Home/Office employees with telephone equipment appropriate to meet the business requirements for their position/job.
- Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.
Driving
This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100/300/100 limits.
TB
This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$69,800 - $96,200 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
