Job description

This position is responsible for providing virtual oversight of patients participating in home-based Remote Patient Monitoring (RPM) programs. This includes collaborating with the patient's provider, caregiver, or other care team members for the provision of services and seamless care based on the unique needs of the patient and providing patient education, support, and interventions via telephone and/or video. In addition will work with the patient to activate and use assigned at-home devices, comprehensively understand their medical, behavioral, and social needs, and co-create a care plan in tandem with the patient, Nurse Practitioners and Medical Directors to help the patient reach their goals.

This role…

  • Establishes vital sign parameters for assigned patients per established clinical models, policies and protocols.
  • Ensures documentation supports administrative functions such as quality management and billing.
  • Follows escalation protocols to appropriately notify other clinical care team members of changes in patient condition or status.
  • Develops and maintains rapport with patients, caregivers, and care team members via phone and/or video communication.
  • Assesses and evaluates patient data and needs, providing appropriate support and education.
  • Identifies the need for and recommends appropriate programs, services, or community resources to promote optimum patient care.
  • Maintains clinical skills within nursing practice norms.
  • Demonstrates a professional attitude, sound judgment, and continuity in interactions with physicians, other healthcare providers, patients and caregivers.
  • Participate in other projects or duties as assigned.

We are interested in speaking to individuals with the following…

  • Associates Degree in Nursing Science required. Bachelor's Degree preferred.
  • Compact State Licensure required (RN Compact).
  • Current and unobstructed RN Licensure in Georgia required.
  • Three (3) plus years of clinical experience required preferably in acute or ambulatory care.
  • Experience working successfully in a fully virtual environment.
  • Experience working with the senior population in home health, clinics, or other health service setting a plus.
  • Or equivalent combination of education and/or experience.
  • Proficient in nursing processes and procedures.
  • Outstanding clinical acumen.
  • Demonstrates empathy, patience, and a strong commitment to providing exceptional patient care.
  • Excellent written, verbal, and interpersonal communication abilities.
  • Proficient in EHR/CMS systems.
  • Displays a highly personable and engaging telephone demeanor.
  • Highly organized with adept prioritization skills.
  • Capable of working efficiently with minimal supervision.
  • Collaborative team player.

Salary: $80,400 - $108,600

This role is bonus eligible based on individual and company performance.
