Job description

Job Overview

As a Nurse Practitioner, you will play a critical role in transforming the lives of patients experiencing chronic kidney disease (CKD) and end-stage renal disease (ESRD). This remote position empowers you to deliver personalized and empathetic care, enhancing the quality of life for your patients.

Key Responsibilities

  • Conduct thorough health evaluations and annual wellness assessments.
  • Provide education to patients and their families on disease management and available support resources.
  • Collaborate with primary care providers, specialists, and a multidisciplinary healthcare team to coordinate care.
  • Ensure accurate and timely documentation of patient records.
  • Prescribe medications, initiate diagnostic tests, and work closely with patients’ physicians.
  • Implement evidence-based practices to improve patient outcomes and minimize preventable hospital admissions.
  • Engage in integrated care team meetings to enhance collaboration.
  • Support the enrollment of patients in relevant educational initiatives.
  • Undertake additional duties as required.

Required Skills

  • Active and unrestricted licenses as a Registered Nurse (RN), Nurse Practitioner (NP), or Physician Assistant (PA).
  • Board certification appropriate to licensure (NP: ANCC/AANP; PA: NCCPA).
  • Current and valid DEA certificate.
  • Ability to work autonomously without direct supervision.
  • Proficient in utilizing medical instruments, equipment, and technology.
  • Excellent verbal and written communication abilities.
  • Strong knowledge of federal and state regulations, including HIPAA and OSHA compliance.
  • At least 2 years of direct patient care experience, preferably in a Managed Care/IPA/Health Plan setting.
  • Experience in conducting comprehensive wellness visits, either virtually or in-home.
  • Bilingual (English/Spanish) is highly desirable.

Qualifications

  • Minimum of 2 years of direct patient care experience.
  • Relevant nursing or medical qualifications based on licensure.
