Job description

Hi, we're Oscar Medical Group. We’re hiring an Licensed Vocational Nurse or a Licensed Practical Nurse to join our Primary Care team.

To help us achieve that goal we are looking for innovative leaders who think big and push boundaries to refactor healthcare and the healthcare delivery system.

About the role:

You will provide and coordinate care for Oscar members in a virtual practice. You will work with a care team of Physicians,  Advanced Practitioners, other Nurses, Social Workers, and Medical Assistants to provide collaborative care.

You will report into the RN Manager.

Pay Transparency:

The pay for this role is: $30 per hour. You are also eligible for employee benefits, and monthly vacation accrual up to 10 days per year.

Responsibilities:

  • Manage patients’ healthcare needs through traditional phone calls, video calls, and asynchronous messaging
  • Maintain quality standards on all documentation and communications using the appropriate platforms.
  • Communicate with clinicians and care team members frequently regarding patient care and escalate cases
  • Perform transition of care outreaches to facilitate post-discharge goals 
  • Provide clinical assistance to determine appropriate support based on patient’s health needs
  • Identify and engage barriers to achieving optimal member health and apply strategies to reduce patient risk
  • Assist clinicians with administrative paperwork, fax orders or prescriptions to hospitals, pharmacies, or other vendors and schedule patient appointments
  • Suggest services or preventive screenings relevant to each patient and coach patients  to reach their care goals by following medical treatment plans
  • Manage pre-visit offerings by patient type and disease status 
  • Support development of expanded nursing services.
  • Follow all member privacy and confidentiality standards of practice (i.e. HIPAA)
  • Compliance with all applicable laws and regulations
  • Other duties as assigned

Requirements:

  • Graduate of an accredited School of Practical/Vocational Nursing (LPN, LVN)
  • Active, unrestricted LVN/LPN license in one of the following states: Georgia, Florida, Texas or compact/multistate license
  • Experience in virtual based practice 
  • Spanish language fluency
  • 3+ years of in person outpatient or ambulatory LVN/LPN experience

Bonus points:

  • Experience in basic behavioral health interventions used in primary care 
  • Experience in healthcare, medical management, change management, and/or leadership
  • Working knowledge of Milliman Care Guidelines
  • Knowledge or familiarity with the principles of project and program management
  • Willingness to obtain additional state licenses as needed

Pay Transparency:  Final offer amounts, within the base pay set forth above, are determined by factors including your relevant skills, education, and experience.
