Hourly Pay Rate: $23
In collaboration with the Accounting Coordinator, perform various tasks to ensure compliance and approvals within the Housing Subsidy Departments to allow individuals to live independently in the community and maintain tenancy.
ESSENTIAL FUNCTIONS/RESPONSIBILITIES:
• Complete QA Checklist in a timely manner in accordance with state guidance
o Follow up with landlords and/or other members of the Circle of Support as needed on items of concern
• Obtain Participant Agreement in a timely manner in accordance with state guidance
• Monitor lease/renter insurance for expiration dates, work with Care Managers and Circles of Support to obtain updated documents
• If no natural supports, assist as needed with transition stipend activities
• If no care manager:
o Assist with finding a place to live
o Assist with lease approval and signing
• Primary department representative at Life Plan Meetings if individual does not receive Community Habilitation
• Assist teammates with tasks as needed, particularly during the annual renewal process. This may include follow up with Care Managers, Circles of Support, and/or OPWDD.
• Reviews Transition Stipend applications if applicable submit to OPWDD for review/approval
• Follows steps per policy to complete steps for approved Transition Stipends.
• Review housing billing before accounting coordinator submission.
• Process Housing payments out of eVero each month and submit payments to A/P for processing
• Creating sending letters for termination cases to Landlord/CC and individual (Per state)
• Obtain any additional items NYS requests for terminations.
• OTHER
o Remote work position, with assigned office time
o Assist with sorting and distributing team mail as needed
o Complies with all agency policies and procedures
o Other duties as assigned
MINIMUM QUALIFICATIONS:
• Associates degree with 2 years’ experience
• Basic knowledge of home inspections
