Required Skills

  • Active and unrestricted RN license in your state of residence (compact license if applicable).
  • A minimum of 3 years of clinical experience.
  • At least 2 years of experience in case management, discharge planning, or home healthcare coordination.
  • Exceptional communication and interpersonal skills with a proven ability to build relationships.
  • Resourceful with a proactive approach to problem-solving.
  • Strong organizational skills and meticulous attention to detail.
  • Proficient in using technology and navigating various software applications.

Qualifications

  • Registered Nurse (RN) with a valid and unrestricted license.
  • 3+ years of clinical experience.
