Overview

Join our team as an Enterprise Population Health Architect, where you will play a vital role in enhancing healthcare delivery through effective population health registries and data models. These tools are essential for ensuring that the right care reaches the right patients at the right time, serving as powerful cohorting instruments that aggregate payer and outcome data to showcase value-based impacts. You will help drive healthcare innovation by managing complex, interconnected processes and utilizing your expertise to identify patients who will benefit from clinical interventions, including new labs, procedures, medications, and follow-ups.

Key Responsibilities

  • Lead the design, building, testing, and implementation of registries and data models within the clinical application system.
  • Provide exceptional support to clinical users by understanding their needs and adapting software to enhance clinical processes, documentation, workflows, and practice standards.
  • Work closely with clinicians to prepare detailed specifications that encompass clinical processes, information flow, and risk and impact analysis.
  • As a technical leader in population health, apply your extensive industry knowledge to manage medium to large-scale projects that have broad scope and complexity.
  • Offer troubleshooting and maintenance support, ensuring user satisfaction and operational efficiency.

Minimum Qualifications

  • A minimum of seven (7) years of related experience, or a Bachelor's degree in a relevant field plus three years of related experience.
  • Proven experience working with clinical application systems, population health, and healthcare analytics.
  • Certification in the assigned EMR vendor’s product/module required within the first six months of employment.
  • Professional-level experience with clinical information systems and report development.
  • Intermediate proficiency in SQL coding, including familiarity with views, automation, stored procedures, and/or SSIS.
  • Strong project management skills and the ability to thrive in group settings and collaborate on projects.
  • Excellent interpersonal, verbal, and written communication skills, including the ability to manage multiple projects and meet deadlines effectively.

Preferred Qualifications

  • Intermediate experience with Tableau or similar data visualization tools.
  • Extensive experience and working knowledge of Microsoft Office products and database management technologies.
  • Proven success in information systems implementation, development, and support within a large-scale healthcare organization.
  • EPIC Certification in Clarity Clinical Data Model is a plus.
  • Familiarity with quality programs such as QIP, EHPI, eCQM, and Magnet.
  • Knowledge of standard healthcare terminologies such as LOINC, SNOMED, CPT, and ICD.
  • Experience in developing and managing knowledge management, data governance, and/or master data management programs.
  • Familiarity with the OMOP Common Data Model and/or Data Warehouse schemas.

Compensation

  • Annual Full Pay Range: \(94,400 - \)176,800 (salary will be prorated if the appointment percentage is less than 100%).
  • Hourly Equivalent: \(45.21 - \)84.67.
  • Factors determining appropriate compensation include experience, skills, knowledge, education, licensure and certifications, and other organizational needs.
