We are seeking a credentialed Nurse Practitioner to join our team as a Clinical Services Coordinator specializing in Remote Patient Monitoring (RPM) and Chronic Care Management (CCM). The ideal candidate will have a strong clinical background, exceptional organizational skills, and a passion for improving patient outcomes through innovative healthcare solutions.

Responsibilities:

Oversee and manage patient care plans for CCM services.
Conduct comprehensive assessments and develop individualized care plans.
Monitor patients’ health status and respond to alerts from RPM systems.
Provide ongoing education and support to patients and their families.
Ensure compliance with clinical protocols and guidelines.
Conduct regular reviews of patient data and make necessary adjustments to care plans.
Serve as a primary point of contact for patients enrolled in RPM and CCM programs.
Facilitate communication between patients and healthcare providers to ensure continuity of care.
Utilize the company portal, electronic health records (EHR), and other digital tools to document patient interactions and track health outcomes.
Analyze patient data to identify trends and areas for improvement.
Prepare and present reports on program effectiveness and patient outcomes.
Contribute to the development and enhancement of RPM and CCM programs.
Participate in quality improvement initiatives and contribute to the achievement of organizational goals.
