Position Summary 

The Intake Coordinator for NYAP’s La Jornada (The Journey) Program focusing on Long-Term Foster Care (LTFC) for minors who have fled their country of origin and arrived in the U.S. without a parent or legal guardian, is responsible for coordinating placement of minors into long term foster care.  

RESPONSIBILITIES 

The Intake Coordinator will perform duties including, but not limited to: 

  • Performing all work in a manner consistent with the National Youth Advocate Program’s mission, values, and philosophies. 

  • Responsible for following the placement procedures, interagency correspondence and required agency and federal database case file information. 

  • Reviewing and updating the referral and case file information in the government database for each minor. 

  • Opening and maintaining case files for each minor in NYAP’s electronic health record system. 

  • Collaborating with multiple internal and external stakeholders to understand foster parent dynamics and ensure minors are placed in the least restrictive environment, based on assessment results. 

  • Communicating with foster families routinely, verbally and in writing. Fluency in speaking and writing English and Spanish is vital. 

  • Collaborating with the Case Management team. Communicate intake specifics about minors’ intake and write and upload progress notes into NYAP’s electronic health record system and the government’s database. 

  • Compiling data and distributing statistical reports to supervisors detailing intake service reception activities. Expert use of electronic database reporting and Excel is essential.  

  • Following agency procedures and be flexible to provide weekend and holiday coverage and be available to potentially work a non-traditional work schedule. 

  • Being a self-starter, well organized and able to work with minimal supervision. This is a work-from-home position.  

  • Providing excellent customer service skills with a focus on caring for people, connecting communities, and promoting peace. 

  • Being a polished communicator responsible for interacting effectively in both English and Spanish with internal and external stakeholders. 

  • Ability to multitask while maintaining high attention to detail. 

  • Performing other duties as requested. 

MINIMUM QUALIFICATIONS 

  • A Bachelor’s Degree in a social service or related field. 

  • Minimum two years’ experience in youth services, preferring direct experience in child welfare. 

  • A valid social work license is preferred but not required. 

  • Written and verbal bilingual English/Spanish fluency is required. 

  • Provide exceptional customer services with a focus on caring for people, connecting communities, and promoting peace. 

  • Advanced use of desktop and laptop computers, internet search, people management programs, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.
