Overview:

Physician Affiliate Group of New York (PAGNY) is adding a Child and Adolescent MHSC Consulting Psychiatrist to NYC Health + Hospitals/Metropolitan's Department of Psychiatry. The Department is at the forefront of providing compassionate and comprehensive mental health services in a dynamic and diverse urban setting. We serve a vibrant community in New York City, offering a wide range of psychiatric services to individuals and families from all backgrounds across our 4 adult inpatient psychiatric units, 1 Adolescent inpatient psychiatric unit, our psychiatric emergency department, our substance use disorder services and robust adult and child ambulatory programs that deliver in-person and telehealth services. Our multidisciplinary team is dedicated to innovation and excellence, providing a supportive environment for both patients and staff.


The hospital has a psychiatry residency program and maintains a strong academic affiliation with New York Medical College to continue its high standards of healthcare delivery, and to provide its staff with teaching and research opportunities.

 

The Mental Health Service Corps (MHSC) is a shared program between the Office of Population Health and Office of Behavioral Health. MHSC is an exciting opportunity to providing a training ground for early career social workers in diverse health care practices across NYC Health + Hospitals earning valuable clinical experience while receiving ongoing training and structured supervision.

MHSC in primary care focuses on the recruitment, training, and support of early career social workers to become fully licensed behavioral health clinicians with a specific focus on incorporating evidence-based practices in primary care settings throughout NYC Health + Hospitals.

30 trainees are placed in primary care practices throughout the system, providing integrated behavioral health treatment through the Collaborative Care model, a team-based care model whose goal is to expand access to behavioral health treatment where services may not otherwise be available. Placements may be in adult primary care, pediatrics, geriatrics, HIV services or women’s health.

Responsibilities:

We are seeking a MHSC Consulting Psychiatrist (Child & Adolescent) in the Office of Behavioral Health and Office of Population Health.

 

Reporting to Medical Director of Outpatient Access in OBH, and the Directors of Psychiatry at assigned facilities, the MHSC Child & Adolescent Consulting Psychiatrist is responsible for supporting behavioral healthcare in primary care clinics using the Collaborative Care model, an evidence-based, treatment to target model of care that uses systematic monitoring of symptoms and a stepped care approach to treatment of common conditions such as depression and anxiety. The psychiatrist will be one member of the Collaborative Care team, which includes a Collaborative Care Clinician (social worker) and a primary care physician, at several clinics across Health + Hospitals.

  • Will rotate amongst different facilities providing Consulting Psychiatry support to primary care teams (e.g. PCPs, Collaborative Care social workers and nurses).
  • Provide regularly scheduled (weekly) caseload consultation to the Collaborative Care Clinicians (social workers) focused primarily on patients who are new to the caseload or who are not improving as expected under current treatment plan
  • Work with Collaborative Care Clinicians to oversee the status and population health of their assigned patient caseload. The psychiatrist consultant assists the Collaborative Care Clinicians in prioritizing patients for case review and treatment adjustment by reviewing the patient registry
  • Recommend treatment adjustments through the case review process or in ad hoc consultations, including medication recommendations and other interventions
  • Document treatment recommendations, as needed, in the medical record
  • Advise on treatment for patients who may need more intensive or more specialized mental health care, supporting treatment in the medical setting until patient can be engaged in specialty care as appropriate
  • Be available to provide ad hoc telephone consultation to medical providers for complex patients or challenging patient situations via phone
  • Provide some direct evaluation and ongoing treatment support of patients with significant diagnostic or therapeutic challenges who are identified in discussion with the Collaborative Care Clinician and/or primary care provider and document appropriately in the medical record
  • Development and delivery of in service training for primary care providers and staff
  • Assist primary care clinics on workflow management and practice transformation toward a Collaborative Care model
  • Credentialing will be done at Metropolitan Hospital -Candidate will be consulting on children and adolescents at : Lincoln, Met, Harlem and South Brooklyn and be consulting on Adults at Harlem (This is ALL remote for all the sites) 
    Hybrid Schedule - 55 Water Street will be Home address - 3x per week in person - 2x a week remote. 

 

Qualifications:
  • MD or DO from an accredited medical or osteopathy school
  • Completion of ACGME accredited psychiatry residency program
  • Board certified or eligible in Child & Adolescent Psychiatry
  • NYS License and DEA Certificate
  • Excellent communication skills
  • Demonstrated ability to collaborate effectively in a team setting
  • Able to quickly synthesize medical and psychiatric data and formulate effective and evidence-based clinical recommendations

Preferences

  • Experience with Collaborative Care and/or consultation-liaison psychiatry
  • Bilingual Spanish preferred
