Job description

Your mission:

As a Clinical Commissioning Manager, you will play a crucial role in our mission to improve health and care services for the community. Tasked with providing clinical leadership and management of the Continuing Healthcare (CHC) including leading the day-to-day clinical service; verification of outcome decisions; individual case management, scheduling support, complaint responses.

The ideal candidate will be responsible for ensuring that professional nursing standards are maintained and that adult CHC nurses meet and maintain professional registration requirements, including mentoring, clinical supervision, and clinical re-validation.

You will also deputise for the Clinical Business Manager as and when required.

Your expertise:

  • Empower the clinical team through strong leadership and management to ensure effective verification, assessment, and management of all CHC eligible adults.
  • Take a leading role in the recruitment and onboarding of new clinical staff, fostering a skilled and dedicated team.
  • Step in as the Deputy for the Clinical Business Manager when necessary, demonstrating competence in decision-making and problem-solving.
  • Handle complaints investigations for the clinical team, providing timely information to the ICB Complaints Manager and conducting sensitive discussions with families as needed.
  • Drive the personalization agenda within the Adult Continuing Health Care framework, advocating for individualized care plans.
  • Provide oversight and leadership for audit and quality improvement initiatives within the CHC Team, ensuring continuous enhancements to service delivery.
  • Implement positive changes within CHC, promoting a culture of reflective practice to benefit both the team and patients and their families.
  • Prepare high-quality, accurate reports and presentations for meetings, including performance data analysis and outcomes of cases, appeals, and retrospective assessments.
  • Offer expert clinical advice and support to team members, patients/caregivers, and senior leads in the ICB, Local Authority, and NHS England.
  • Foster collaborative and professional relationships with CSU, ICB, and external stakeholders to facilitate seamless healthcare delivery.
  • Approve high-cost care packages, verification panels, and individual placement agreements, ensuring adherence to processes and policies.
  • Agree on financial expenditures within delegated limits for eligible packages, following the ICB's Choice, Resource, and Allocation Framework.
  • Review complex cases to ensure clinically appropriate and cost-effective care plans, making financial decisions on behalf of the ICB within delegated limits.
  • Identify and conduct audits, investigations, and surveys regularly to enhance clinical practice and care provision.
  • Stay up-to-date with national research to inform personal and organizational development and support evidence-based practices.
  • Drive the implementation of the ICB's QIPP agenda in relation to personalized care.
  • Support the establishment of clinical governance arrangements, including audit processes.
  • Utilize expert clinical knowledge to verify adult continuing healthcare recommendations.
  • Provide operational management and leadership to the CHC nursing team, ensuring consistent, high-quality delivery of national policies and the NHS Adult Continuing Healthcare Framework.
  • Effectively line manage the Band 7 CHC Complex Case Managers and Occupational Therapist, overseeing performance, conducting appraisals and development plans, and addressing disciplinary or capability issues when required.
  • Support, motivate, and develop nursing staff within the team, equipping them with the necessary skills and knowledge for excellent CHC practice.
