Your mission:

As a vital member of our Learning Disability (LD) Team, you will have a unique opportunity to make a difference in the lives of vulnerable individuals within our community. Your role as a Nurse Assessor will be instrumental in ensuring the delivery of high-quality care and contributing to cultural change to achieve outstanding results.

This is an exceptional chance for a motivated and experienced registered nurse, ideally with an RNLD qualification or substantial LD field experience. You will use your assessment skills to support and advocate for some of the most vulnerable individuals, contributing to the assessment and review processes for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC), including the promotion and evaluation of Personal Health Budgets.

Your expertise:

  • Facilitate multi-disciplinary assessments and patient reviews following our standard operating policies and procedures. This includes conducting face-to-face and virtual assessments/reviews.
  • Support the Complex Case Manager/Clinical Commissioning Manager and the wider team to ensure effective planning and management of workstreams, ultimately achieving successful delivery.
  • Manage a clinical caseload from end to end, resolving issues related to funding, placements, invoices, and complaints, all while adhering to Continuing Healthcare guidance and maximizing patient safety.
  • Assist in the development of personalization initiatives across the organization, promoting a patient-centred approach to care.
  • Provide accurate and detailed recording of information, maintaining comprehensive and legible records.
  • Offer guidance and support to individuals and their representatives throughout the NHS Continuing Healthcare process.
  • Enhance management information and reporting to support data-driven decision-making processes.
  • Contribute to the development of performance and governance strategies and actively participate in improvement programs.
  • Deliver against objectives and performance targets, prioritizing your workload, and working efficiently within tight deadlines.
  • Support the training and induction of new staff, sharing your knowledge and expertise.
  • Undertake research and development as directed to stay updated on best practices and advancements in the field.
